I'm working on a mod for M&B Warband, and mumble works remarkably easy with it, for directional audio. One major hitch is that players that have not spawned can hear everything in a channel. Secondly, each player can decide what his own settings for the directional audio are.

Is it relatively easy to create a custom client, then host it, that fixes these two annoying things?

No, it is not easy. Or well, it is, because you just have to limit the settings the user has access to.
